What Is Deep TMS (dTMS)? How It Works and Who It Can Help

Understanding dTMS: A Deeper Form of Brain Stimulation

Deep Transcranial Magnetic Stimulation (dTMS) is a non-invasive mental health treatment that uses magnetic fields to stimulate deeper areas of the brain than traditional TMS. It’s FDA-cleared for major depressive disorder (MDD), obsessive-compulsive disorder (OCD), and smoking cessation.

Unlike standard rTMS, which targets surface-level brain regions, dTMS uses a specialized coil called the H-coil to reach neural circuits located deeper in the brain’s cortex.

How Does dTMS Work?

dTMS works similarly to traditional TMS, delivering magnetic pulses to stimulate underactive parts of the brain involved in mood and behavior regulation. However, the key difference lies in the coil design and treatment depth:

  • Traditional TMS (rTMS): Reaches ~1.5 cm into the brain
  • Deep TMS (dTMS): Reaches ~4–5 cm into the brain

This added depth allows dTMS to target broader and deeper neural networks, which may be especially beneficial for individuals with complex or treatment-resistant symptoms.


FDA-Cleared Uses of dTMS

The U.S. Food and Drug Administration (FDA) has cleared specific dTMS protocols for:

✅ Major Depressive Disorder (MDD)

dTMS was first FDA-cleared in 2013 for depression. It is typically used when patients haven’t responded well to at least one antidepressant medication.

✅ Obsessive-Compulsive Disorder (OCD)

In 2018, dTMS received FDA clearance for OCD using a different coil and protocol (H7 coil). It targets deeper circuits involved in compulsive behavior and intrusive thoughts.

✅ Smoking Cessation

As of 2020, dTMS is also cleared to help adults quit smoking, by stimulating reward pathways and reducing nicotine cravings.


What Does a dTMS Session Look Like?

A typical course of dTMS involves:

  • Daily sessions (Monday–Friday)
  • Each session lasts about 20–30 minutes
  • A cushioned helmet with the H-coil is placed over the head
  • You remain awake and alert
  • No sedation or recovery time is required

Some patients experience mild scalp discomfort or headaches, but these are usually temporary. Serious side effects, such as seizures, are very rare.

Who Might Benefit from dTMS?

dTMS is typically considered for individuals who:

  • Have not responded to medication or psychotherapy
  • Need non-systemic (non-drug) treatment options
  • Are looking for FDA-cleared alternatives for depression, OCD, or nicotine addiction
  • Have complex brain activity that may require deeper stimulation

It is not recommended for individuals with metal implants, epilepsy, or unstable medical conditions without proper medical clearance.


What Does the Research Say?

Clinical trials and real-world studies have shown that dTMS can lead to:

  • 60–70% response rates in patients with treatment-resistant depression
  • 30–40% remission rates, comparable to traditional TMS
  • Effective reduction in OCD symptoms using the H7 coil
  • Statistically significant outcomes for smoking cessation

One 2015 study published in World Psychiatry found that dTMS significantly reduced depression symptoms within 4 weeks, even in patients who had tried multiple medications.


Is dTMS Covered by Insurance?

Yes, many insurance plans cover dTMS for depression and OCD, especially when prior treatments have failed. Insurance for smoking cessation with dTMS is more limited and may vary by provider.

Always check with your insurance provider or the treating clinic to verify coverage and pre-authorization requirements.
